I Who will decide what type of health services I should be enrolled into?
It is usually directed by your physician. However, if you have not been formally prescribed a home health care program, you can set an appointment with us for a health needs assessment.

What are the qualifications to receive Adult Care Services?
Eligible clients include home-bound individuals, patients who have recently been discharged from the hospital and clients who have been prescribed by their physician to be recipients of adult home health services.
